What is the "Caring for Your Diabetes" program?
"Caring for Your Diabetes" is a community diabetes care model that uses trained Diabetes Care Managers to work with other healthcare providers, including your primary care physician, and diabetes educators. The goal of this collaborative effort is to improve disease management for people with diabetes, improve health outcomes and help control care costs.
Who is eligible to participate?*
Any Team Member or covered dependent who is enrolled in Mountain States' Medical and Pharmacy benefits.
What do I have to do to participate?
You will be asked to sign a consent form in which you agree to participate in the program and give permission for release of medical information. You will visit your Diabetes Care Manager on at least a monthly basis. You will also be required to attend one of Mountain States' Diabetes Educational Programs.
How will this benefit me? What will it cost me?
Because the management of diabetes can be very complicated, the program will be set up to help you manage this illness. Your Diabetes Care Manager will spend time with you to review your health needs, answer your questions and follow up on problems with your doctor. A major goal of this project will be to provide good communication of information about your health needs to all your healthcare providers and improve your care. You will have the co-payments waived for all of your diabetes-related prescriptions and supplies,** including a glucose monitoring machine. You pay nothing for the extra services.
Do I have to change physicians?
NO. Please continue to see your physician at regularly scheduled intervals.
How often do I have to see the Diabetes Care Manager for this program?
At least once a month. You may see or call him/her more often if you like.
How do I get started in the program?
Please complete the attached form and return to your Human Resources Department. The program coordinator will contact you with additional information and connect you with your Diabetes Care Manager.
