Employee Assistance support staff need certain information to properly evaluate and refer you to the appropriate professional. This information may include:
- Name, address and telephone number - your name and phone number are needed so the counselor can get back to you.
- Medical coverage - information about your medical coverage may help the counselor determine what is best for you.
- Employment information - information about your job, such as job title, length of service, duties and so on, may prove helpful to the counselor's initial assessment.
- A brief description of your problem.
