Therapy Costs

Your costs for mental health and substance abuse services provided by the EAP are listed below. Not included in this program are the cost of

medications and outpatient lab work or any services related to the treatment of sexual offenders, long-term personality change/growth therapy or autism.

Pre-Certification  

The first 12 visits are without a co-pay when rendered through Mountain States Psychiatry. All visits beyond the 12th require a $25 co-pay per visit.

Out-Of-System Services 

Any out-of-system outpatient services must be preapproved by the EAP manager. This will only be done when the EAP believes it is more expedient and clinically appropriate for these services to be provided out-of-system or for those who live outside the area. Each visit must be pre-approved and authorized with a pre-determined rate negotiated by the EAP with the provider. 

Out-of-system emergency admissions to inpatient facilities will be covered at a maximum of three (3) days per incident and a total of six (6) days in a lifetime. 

Any covered days beyond these limits must be preapproved by the EAP. Where it is believed that continued hospitalization or partial hospitalization is needed, the EAP will negotiate the provision of this service.

(There is a $275 co-pay per admission.)

The objective will be to get Team Members into the EAP system for any inpatient or partial hospital services.  Virginia Team Members may be referred to resources in their area and will still receive the same services and rates.
