Pre-tax Healthcare Account: Benefit-eligible Team Members may sign up for pre-tax savings to cover deductibles, co-pays and other medical expenses. Team Members access this account by using a SmartFlex Debit Card.
Pre-Tax Dependent Care Account: Benefit-eligible Team Members may sign up for pre-tax savings for daycare expenses.
Diabetes Management Program: Benefit-eligible Team Members and their covered dependents can join the diabetes program to help manage their diabetes. This program offers education and free selected supplies and prescriptions.
Accident and Critical Illness: Voluntary benefit for benefit-eligible Team Members allows planning for potential accidents and illnesses.
Short-Term Disability: Disability coverage for benefit-eligible Team Members that provides Team Members with a portion of pay while out of work.
Long-Term Disability: Disability coverage for benefit-eligible Team Members who are unable to work because of a qualifying disability due to an accident, injury or illness. Benefit pays after 180-day waiting period.
Long-Term Care: The type of assistance needed if you are unable to care for yourself because of a prolonged illness or disability, from daily activities at home to skilled care in a nursing home. This benefit is available to benefit-eligible Team Members and their spouses, parents (including in-laws) and grandparents (including in-laws).
Life Insurance Accidental Death and Dismemberment Insurance: Benefit-eligible Team Members can provide financial support to beneficiaries in the event of death.
Dependent Life Insurance: Benefit-eligible Team Members can ensure financial support in the event of the death of a dependent.
Supplemental Life Insurance: Additional life insurance available to benefit-eligible Team Members.
Retirement Savings Plans: 401(k) plan available for retirement savings. Team Members working at a not-for-profit entity will be enrolled in the pension plan after working one year, working 1,000 hours and reaching age 21. Team Members working at a for-profit facility may enroll in a retirement savings plan during open enrollment dates of Jan. 1 or July 1 after reaching age 21 and working at least 1,000 hours.
Benefit Waiver: Benefit-eligible Team Members may waive all benefits and receive a 20% differential for not choosing benefits.
Adoption Assistance: Adoption assistance is available for benefit-eligible Team Members for up to $5,000 per adoption for qualified legal and adoption agency expenses as limited in the policy.
Comprehensive Team Member Assistance Program: Provides benefit-eligible Team Members and their covered dependents with confidential mental health and substance abuse services. The first 12 visits are free each plan year.
Team Member Emergency Fund: All paid Mountain States Team Mmbers are provided a confidential mechanism for seeking financial assistance when confronted with personal emergencies.
Bereavement Leave: Full-time, benefit-eligible Team Members receive 3 days off for the death of an immediate relative and one day off for the death of a grandparent or grandchild after 90 days of employment. Part-time, benefit-eligible Team Members receive one day off for the death of an immediate relative after 90 days of employment.
PTO Donation Plan: Benefit-eligible Team Members with a minimum of 120 hours in their PL/PTO accrual bank may donate hours to Team Members in crisis. Donations can be made to the Team Member Emergency Fund or directly to a Team Member using the PTO cash-out form.
PTO Cash-Back Option: Benefit-eligible Team Members have the opportunity to receive cash or retirement funds in lieu of some accrued paid time off once they have 120 hours in their bank.
Child Development Centers: On-site child care is available at both JCMC and SSH facilities. Team Members receive highest priority for available openings.
Wellness Center: Serves the community as well as Team Members and their families, offering cardiovascular equipment, free weights, circuit weight training, equipment, variety of fitness classes, heated therapy pool, saunas and indoor lap pool.
Paid Time Off: Paid time off is provided to benefit-eligible Team Members for rest and relaxation. The amount of time will be accrued annually by Team Members. Accrued, unused time may be paid out at termination of employment if employed more than 90 days.
Major Medical Leave: Major medical leave is provided to benefit-eligible Team Members for protection against the potentially negative financial impact of absence due to personal illness or injury. The amount of time will be accrued annually.
Leave of Absence: Available to benefit-eligible Team Members after designated amount of time for medical, maternity, FMLA, personal and military leave.
Tuition Reimbursement: After six months of employment, benefit-eligible Team Members are eligible for assistance when returning to school for a graduate or undergraduate degree. Classes must be related to your current job or qualify you for a different job with Mountain States. Team Members who benefit from this program will incur a service obligation to Mountain States.
Referral/Recruitment Bonus: Eligible positions and the bonus amounts for each position are published on the Mountain States and HR intranet home pages and are updated regularly.
MSHA University: All facilities offer programs sponsored by the MSHA University to provide Team Members with professional development opportunities in many professions and occupations.
Christmas Gift Certificates: Provided annually to all Mountain States Team Members at their facility's Christmas party.
Discounted Meals: Available to all Mountain States Team Members who have a cafeteria in the facility in which they work.
Credit Union: Team Members are eligible to become members in the credit union immediately upon employment with Mountain States. Online banking is available.
Service Awards: Beginning at five years of continued service and then at five-year increments, Team Members are recognized at an annual program and presented with a service pin.
Stars Program: Team Member recognition program designed to spotlight top-performing individuals and teams. Star Cards are redeemed for prizes.
Online Classified Advertisements: Team Members can advertise items for sale or rent, wanted items and professional services and may also look for items to buy online using the Mountain States intranet.
Discounts to local businesses: A variety of discounts are available and updated regularly online. Some discounts include movie tickets, Biltmore Estate tickets, dinner shows in Pigeon Forge, cell phones and computers.
