The vice president or senior vice president. If endorsed, it should then be submitted to:
The president and CEO for approval.
The nomination should include: Team Member name, description of implemented improvement, outcomes of improvement, suggestion for award level, signatures of nominating supervisor, director, VP/senior VP.
