Why are we doing this?

Fit 4 Life provides a supportive work place for you to become the best that you can be. The Fit 4 Life program is your “scorecard” as you work toward that better you. You benefit by leading a healthier lifestyle and earning discounts on your future medical premiums. MSHA benefits by having healthy team members with increased productivity and decreased sick time. We all benefit if we can slow the rising cost of our medical plan.

Who is eligible for premium discounts?

All benefit-eligible team members are eligible to collect wellness points in the 2012 calendar year to be used toward a discount on the FY13 medical plan premium. This includes associates not currently in the medical plan, but who may be interested in joining next year.

All MSHA team members are welcome to participate in the wellness programs and activities, whether you plan to participate in the Fit 4 Life program and collect points or not.

How does it work?

Team members in the MSHA medical plan earn points by participating in the Fit 4 Life program. The points you earn in 2012 will determine the discount you receive on your medical plan premium costs in FY13. With a potential of 5,700 points, reaching one of the tiers can be easy. There are three tiers of point totals, and more points means a greater discount on your premium.

Tier 1: At least 3,000 points = $20 per pay period = $480
Tier 2: At least 2,500 points = $10 per pay period = $260
Tier 3: At least 2,000 points =   $5 per pay period = $130

Team members must complete the following three (3) requirements to participate in the program:

  • Routine Annual Physical
  • Complete Health Risk Assessment with MSHA

(Team members must be tobacco free to receive either Tier 1 or Tier 2 discounts.)

How do I get points?

Step 1: Get your Fit 4 Life Program Guide from your HR Department or the HR intranet site.

Step 2: Review the guide for the requirements, measurements, activities and points that will help you understand how to build points toward your incentive.

Step 3: Get the activities form from you HR Department or the HR intranet site.

Step 4: Get started on a healthier you! Don't forget to complete the two requirements at the top of the
scorecard.  The rest of the scorecard is a list of potential opportunities to earn additional points.

Step 5:  Maintain your scorecard throughout the year as you complete the activities.

Step 6: In the last quearter of the fiscal year, the wellness team will announce how to turn in your scorecard to get the financial benefits of your hard work and a reduction in your medical premium!
